New Patient Registration

We have an open list and welcome requests for registration from patients living in or moving to the practice area.  Our car park is adjacent to the surgery and includes  parking spaces for disabled visitors.  There is wheelchair access to the premises.

Public transport: Bus number 324 from Huddersfield town centre.

New patients can register by completing form GMS1 available at reception.

All new patients are now offered a screening check by the Healthcare Assistant.  This is for routine checks, i.e. blood pressure, weight, height etc.

Patients are able to see any of the doctors regardless of who they are registered with.  Patients who express a preference will be given the opportunity to see the doctor of their choice at the next available appointment.

Updating Your Details

If you change name, address or telephone number, please let our receptionists know by calling or writing to us. If you move outside the practice area you may need to find a doctor in your new area.
